Deactivate Your Account

You may deactivate your MyUCSDHealth/MyUCSDChart account. Your login to MyUCSDChart will be removed and you will lose access to the portal and mobile app. Because your information displayed in MyUCSDChart comes from and is stored in your electronic medical record at UC San Diego Health, all information you see in MyUCSDChart including your demographic information, your medical information, your scheduled and past appointments, your messages to and from your physicians, and other associated information will not be deleted from the electronic medical record due to legal requirements and to ensure that you continue to receive the best possible care from your healthcare providers.
On the Web Site:
Click this Deactivate Your Account link, log in to MyUCSDChart, and you will go directly to the "Deactivate Your Account" page. Read the notice there and click the "Deactivate" button to deactivate your account.
If you are already logged in, click the Menu, choose the "Security Settings" menu item, scroll down and click the "account deactivation" link, read the notice, and click the "Deactivate" button.
On the mobile app:
Log in, tap the menu, tap "Account Settings", scroll to the bottom and tap "Deactivate Account", read the notification and tap the "Deactivate" button.
