Welcome to UC San Diego Health System's MyUCSDChart! Your health is important to you around the clock - not just during office hours. With this secure, private connection, you can:

Message your Clinic

Send a non-urgent** health-related message to your clinic. Messages to and from your doctor's office are secure and private. You will be alerted when you have a reply from your clinic.

Access Your Medical History

View your medications, immunizations, documented allergies and other current health issues, including preventive care recommendations based on your age, gender and medical history.

View Select Lab Test Results

Routine results are available for view four days after their result date. You may also view your doctor's comments.

Request an Appointment

Request a non-urgent** UC San Diego Health System doctor's appointment for a time that's convenient for you. View upcoming appointments and access post-visit summaries for past appointments.

Prescription Renewals

If you are out of refills and require a non-urgent** prescription renewal, including a mail-in, secure or written prescription to take to an outside pharmacy, you may make a request through MyUCSDChart.

Messages from Your Clinic

Receive health reminders and read and print letters from your clinic.
