Terms & Conditions for User Access to the UC San Diego Health “MyUCSDChart”

University of California San Diego Health (UC San Diego Health) is pleased to offer you the ability to view parts of your medical record and interact with your physician’s office online by using MyUCSDChart. This web site and related services are subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this Site.

A. Use of MyUCSDChart

NEVER send any messages requiring urgent attention using MyUCSDChart. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume and availability of staff. Therefore, for all urgent medical matters, contact your health care provider's office immediately, go to an emergency room, or call 911.

B. Eligibility

Two requirements must be met for a patient to be eligible to participate in MyUCSDChart. First, you must be 18 years or older. Second, you must be an active patient of a UC San Diego Health health care provider who has elected to offer MyUCSDChart to his/her patients. IMPORTANT NOTE: Not all UC San Diego physicians will choose to communicate using MyUCSDChart, in which case communication will be through US mail service or telephone.

C. Activation Code

MyUCSDChart is designed as a secure Internet-based environment through which you may receive confidential medical information about yourself. Once you sign up for MyUCSDChart, you will be given a one-time-only MyUCSDChart "Activation Code" that you will use to activate your account. You will then create your own MyUCSDChart username, password and password hint. Please DO NOT SHARE your password with anyone. The individual MyUCSDChart username and password you choose when you first log-on to MyUCSDChart are the keys to protecting the confidentiality of your information. You are responsible for remembering your MyUCSDChart username and password and for keeping them confidential.

  1. From the time you receive your temporary, one-time use Activation Code, you will have ONLY 90 days to log-on and activate your MyUCSDChart account. After 90 days, if you have not activated your account, your temporary ten-digit "Activation Code" will expire. PLEASE activate your account as soon as possible so that you may begin using the services immediately, and not risk the inconvenience of having to sign up a second time.
  2. After you have activated your MyUCSDChart account, MyUCSDChart will become the primary vehicle through which you will receive important messages from your primary care health care provider and selected test results and other personal medical information. You will continue to receive communications via the United States Postal Service or other means from health care providers that have opted to not use MyUCSDChart.
  3. Through MyUCSDChart you may request new appointments and prescription renewals 24 hours a day, 7 days a week. We ask that you first contact your pharmacy for existing, non-narcotic, prescription refills. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume and availability of staff. Therefore, for all urgent medical matters, contact your health care provider's office immediately, go to an emergency room, or call 911.
  4. You MUST provide us with your email address when activating your MyUCSDChart account and keep your email address current via the "Personal Preferences" link in MyUCSDChart. No confidential information will be sent to your email, but you will receive email notification whenever a new result has been released to your MyUCSDChart account. You therefore MUST verify and update your email address via the "Personal Preferences" link to ensure that you receive notification of newly released information in a timely manner.
  5. Once you have activated your account, your patient header display will include an active MyUCSDChart designation. You will then receive your personal health information as released by your health care provider in your personal MyUCSDChart account.

D. Electronic Messaging

  1. Your health care provider may choose to select an associate to help manage and respond to your messages. These may include nursing or other administrative staff who review messages sent via MyUCSDChart and either respond or route the message to the health care provider. Therefore use of MyUCSDChart secure messaging may not be appropriate if the message includes sensitive information that you would only wish to discuss directly with your provider.
  2. All communications between you and your UC San Diego Health care team using MyUCSDChart are carried over a secure, encrypted connection directly into the UC San Diego Health electronic medical record. While you may receive Internet e-mail messages notifying you of new messages in your MyUCSDChart Inbox, these e-mails will not contain any confidential medical information.
  3. Messages you send via MyUCSDChart may become part of your permanent legal medical record at the discretion of the physician or support staff. Once a message is made part of your permanent medical record, it will be accessible to current and future UC San Diego Health care provider(s) and staff members who are appropriately involved with your care.
  4. You understand that when you receive messages, test results and other communications delivered via MyUCSDChart, you will not receive duplicate written reports of this information from UC San Diego Health.

E. Medical Record Data

  1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including results of all test, procedures and therapies.

    NOT ALL of your UC San Diego Health medical record may appear on your MyUCSDChart. Posting of information is at the discretion of your individual health care providers(s) and UC San Diego Health.
  2. Your health care provider(s) will maintain and update your electronic record at each office visit. However, if you believe any information displayed in MyUCSDChart is not accurate, you are encouraged to notify your health care provider at your next office visit or submit a message to your clinic.
  3. UC San Diego Health reserves the right in the future to charge a fee for the use of MyUCSDChart. You will be notified of this change and asked to re-apply if you agree to the fee and wish to continue with this service.

F. Information Does Not Constitute Medical Advice

UC San Diego Health assumes no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you. You may be accessing your own medical information via the MyUCSDChart application and may wish to communicate with one of your health care providers about that or related information. Neither MyUCSDChart nor any other application available through this Site is a substitute for appropriate and timely contact with your health care provider. This Site is not a substitute for consultation with your health care provider. You should never change or stop any course of treatment prescribed by your health care provider without first consulting him or her.

G. Termination of Access

UC San Diego Health may restrict or terminate your access to MyUCSDChart at any time. If access is restricted for planned maintenance of the MyUCSDChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyUCSDChart without advance notice. If your use of MyUCSDChart violates these Terms and Conditions, UC San Diego Health reserves the right to permanently terminate your access to MyUCSDChart.

H. Restrictions on Use and Ownership

All pages within this Site and any material made available for download are the property of UC San Diego Health. The content on the Site is provided for UC San Diego Health and its patients only. The words “UC San Diego Health” and its associated logo are the trademarks and service marks of the Regents of the University of California (UC Regents). All other trademarks, service marks, and logos used in this Site, including but not limited to MyUCSDChart (MyChart) and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of UC San Diego Health or the third party that may own the trademark, service mark, or logo.

UC San Diego Health grants a limited license to each user to make personal use only of the Web site and the associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the Web site and the associated services, making any derivative of the Web site or the associate services, the collection and use of user email addresses or other users information, including, without limitation, health information or any data extraction or data mining whatsoever.

I. Accuracy and Integrity of Information

Although UC San Diego Health attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform UC San Diego Health so that it can be corrected. Information contained on the Site may be changed or updated without notice.

J. Security

This Web site uses encryption software. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using MyUCSDChart, information will be transmitted over a medium that is beyond the control of UC San Diego Health and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USER OF THIS SERVICE. Once the information is received by UC San Diego Health, your medication and other medication information will be treated as confidential and given the same protection that all other UC San Diego Health medical records are given.

When you access a non-UC San Diego Health site, please be advised that it is independent from UC San Diego Health, and that UC San Diego Health has no control over the content on that web site.
