Web Site Customer Support

For the following issues, please try these self service links first: Online self sign up, Forgot password?, Forgot username?

If your account is locked from too many failed login attempts, if you are having trouble with online self sign up for a MyChart account, or if you have a technical problem preventing you from logging in call (619) 543-5220. Otherwise, if you are able to log in and have a different question, please log in and send a customer service message there.

Note that this group cannot answer billing, clinical, appointment scheduling, or medical records questions--for those, see below.

Billing Customer Support

Visit Billing Support or call (855) 827-3633. If you have been impacted by COVID-19, ask us what options may be available to you.

Clinical Support

Clinic Contact Information - Clinic and hospital phone numbers, addresses, directions, parking information, and more.

If you think that some aspect your health information represented online is incorrect, or if you have questions about your clinical information in MyChart, please log in and send a message to the office of your physician or contact your clinic directly.

Find a Provider

Search for physicians within our system, including clinic, contact, and biographical information.

Appointment Scheduling

  • Primary Care: Log in and schedule online using MyChart or if you have questions about or need to change a scheduled appointment contact your clinic.
  • Specialties: Obtain a referral from your primary care physician and contact the specialty.
  • Assistance for patients who are deaf and hard of hearing: Log in and use MyChart direct scheduling or the designated Appointment Request Form. Or contact 800-926-8273 (M-F 8a-5p) using TTY 711 or 800-735-2922.
  • Other scheduling questions or help: Call 800-926-8273 (M-F 8a-5p).
  • Do not use MyChart scheduling for urgent matters. For health related emergencies, call 911.

Medical Records

Not all of the information in your personal health record is accessible to you on the MyChart web site. If you need access to information beyond what is included online, are having trouble using MyChart Medical Records Request, or have other questions about about your medical records, please visit Medical Records.
