What is MyUCSDChart?

MyUCSDChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyUCSDChart, you can use the Internet to:

How do I sign up?

Patients who wish to participate will be issued a MyUCSDChart Activation Code during their clinic visit. This code will enable you to login and create your own username and password. If you were not issued an Activation Code during your last office visit, ask to sign up during your next office visit or call MyUCSDChart Patient Support at (619) 543-5220, Monday through Friday, 9AM - 4PM.

When can I see my lab test results in MyUCSDChart?

Your test results are released to your MyUCSDChart account after your health care provider has reviewed and manually released them or automatically after four business days. Note that not all test results are available for viewing online. These include Radiology results (MRI, CT, X-Ray, etc.) as well as results (from any department) considered to be sensitive or requiring prior discussion with your provider. If you do not see expected results within a reasonable period of time, you may call your clinic directly to inquire.

Why are certain lab test results not shared electronically via MyUCSDChart?

If some of my health information on MyUCSDChart is not correct, what should I do?

Your MyUCSDChart information comes directly from your electronic medical record at your health care provider's office. You can send a MyUCSDChart message asking the provider to correct your medical record or call your clinic directly. Alternately, you can have the provider update the information at your next clinic visit.

If I send a message to my health care provider, when can I expect a reply?

You will generally receive an answer within three business days. Please note that MyUCSDChart should not be used for urgent situations. Please contact your provider if the situation requires immediate attention. For emergencies, call 911.

Can I view a family member's health record in MyUCSDChart?

Parents or legal guardians may request proxy access to view their child's record for children ages zero to eleven years old. Ask at your clinic or call Customer Support for more information. Once you have been granted proxy access, you access your child's information by logging into your own MyUCSDChart account.

Can I ask questions regarding a family member from my MyUCSDChart account?

Information communicated about another family member through your online account might, for technical or legal reasons, remain in your record only. If you have health questions or information about another family member, please discuss these directly, in person or by phone, with your provider so that pertinent information may also be updated in your family member's health record if necessary. Alternately, if you have proxy access to your child's account as described above, you may ask questions on their behalf from within their account.

No, due to the sensitive nature of medical information, each adult must establish their own MyUCSDChart account.

Where can I update my e-mail address or change my password?

Log into MyUCSDChart. From the left menu, go to the Preferences section and choose either "Notifications" to change your e-mail address or "Change Password" to change your password.

How is MyUCSDChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure Activation Codes, personal usernames, and passwords. Each person controls their own password, and the account cannot be accessed without that password. Further, MyUCSDChart uses the latest encryption technology to automatically encrypt your session with MyUCSDChart. Unlike conventional e-mail, all MyUCSDChart messaging is done while you are securely logged on to our website.

I was logged out of MyUCSDChart, what happened?

We aim to protect your privacy and security of your information. While logged into MyUCSDChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyUCSDChart. We recommend that you log out of your MyUCSDChart session if you need to leave your computer for even a short period of time.

My Activation Code does not work, what should I do?

For your security, your Activation Code expires after 90 days and is no longer valid after the first time you use it. If you have problems, call MyUCSDChart Patient Support at (619) 543-5220, Monday through Friday, 9AM - 4PM.

Is my Activation Code my username?

No, your Activation Code is not your username or password. You will use this code only once to activate your account. When you activate your account, you will be asked to create your own unique MyUCSDChart username and password.
